- At least 18 years or the age of majority in their respective states or province or country.
- Not an employee of Mark’s and the Sweepstakes Sponsor, Empathica Inc., and any of their immediate family members or person in the same household or anyone associated with the contest.
- Need to visit a participating Mark’s store lately and receive a survey invitation on the cash register receipt.
Marks Customer Satisfaction Survey Contest Entry
There are several ways to enter Mark’s Customer Feedback Survey Sweepstakes program- Online, by Phone or by Mail. If you fulfill all the eligibility requirements required for the survey, you can choose any method according to your preference.

- Visit the Survey URL printed on your Mark’s Survey Invitation. Or, you can go to www.marks-survey.com
- Enter the Survey Code from the bottom of your receipt. (If you have trouble locating the number, take a look at the sample receipt image provided in the middle to find it on your customer copy).
- Click on the yellow “Begin Survey” button to begin the survey.
- Respond to the required survey questions.
Make sure you fill out all the areas marked with an asterisk before submit. When the survey is complete, you will need to provide some of your personal contact details i.e. name, phone number, etc. to enter the sweepstakes.

Besides the online survey, the eligible entrants can take the telephone or SMS Survey and get discounts as well as participate in the sweepstakes for a chance to win prizes. To complete a phone survey, you will need to call the toll-free number on your Mark’s Survey invitation and respond to the recorded prompts.
And for the SMS Survey, text your responses to the telephone number provided in your survey invitation instructions. At the conclusion, you will have an opportunity to provide your Sweepstakes Entry information to enter the applicable Monthly Draw.

After completion of the survey, you can enter into a draw for a chance to win 1 of 2 $500 Mark’s Gift Cards.
Important Notes on Mark’s Survey Sweepstakes:
- All the eligible and complete entries will be entered in the draw for the daily sweepstakes for the day or the following Weekly Sweepstakes in which they are received.
- Random drawing for the daily prizes will be held monthly and weekly prizes will be awarded each week.
- The selected winners will be contacted by telephone or email within 10 days of selection and they must respond within 10 days of the initial notification to claim the prize.
- The prize will be sent to the winner within 60 days of the applicable draw date.
