Forgot S Number

• After that, they need to click on the ‘Forgot S Number’ link at the top right of the page.

• For S Number recovery provide the following data:

1. First name
2. Last name
3. Last 4 digits of Social Security Number
4. Date of birth
5. Select a role that describes you
6. How would you like your User ID delivered to you

• Click the ‘Submit’ button and you can find User ID.

Forgot Password

• In case the user has lost password they need to visit the login homepage of CCD Connect.
• Below the login section, there is a ‘Need Help?’ section. Under it there is the ‘Reset Password’ link.
• The ‘Forgot Password?’ link is also at the right of the login widget.
• Click on it and a new page will open which asks your S Number.
• Enter S Number and click ‘Submit’.
